Hours Tracking Form

Please help us to report on the great work that OSU students have accomplished in our community. Submit this form to track your service hours with the CCE.

Alternatively, you can print, complete, and turn in the appropriate hour tracking form to our office in Snell Hall 158, or drop by our office where we will have everything you need to start tracking your hours.

If you completed these hours as an affiliate of one of these types of organizations, please check the corresponding box so that we can add your hours to the running totals for all organizations of this type. If you are submitting these hours on behalf of multiple organizations, please check all classifications that apply.
